    Microwave ablation of adenomyosis – a conservative uterus preserving procedure.

    About the Patient and Condition

    A 40-year-old lady with painful periods and heavy bleeding came to Apollo Health City, Jubilee Hills, Hyderabad. She was also anxious to conceive.

    Case Scenario

    Patient was diagnosed with Adenomyosis (Uterine volume 400 cc and adenomyoma volume 233 cc) and endometriosis. She was offered LUMWA (Laparoscopic and Ultrasound Guided Microwave ablation) as a uterine preserving conservative procedure along with endometriosis excision. The patient tolerated the procedure and was discharged in 48 hours.

    Microwave ablation has gained increasing attention because of its high thermal efficiency. It also helps reducing symptoms without affecting the ovarian function. Microwave ablation when performed through laparoscopy under ultrasound guidance has shown promising results in symptoms reduction, fertility enhancement and uterine volume reduction.

    Ultrasound and laparoscopy-guided PMWA (Percutaneous MicroWave Ablation) is a feasible minimally invasive technique for the treatment of adenomyosis. It has several advantages and no severe side effects. The uterine volume and the adenomyoma size decreased significantly in the follow-up period. Therefore, transvaginal ultrasound- and laparoscopy-guided PMWA is an effective technique.
