Interventional Cardiology

The Interventional Cardiology Department is manned by trained and dedicated personnel and is one of the busiest in the city. Coronary stenting is routine and performed by most of the interventional cardiologists. Patients with high risk and multi vessel involvement are also treated.

Primary angioplasty, an approach of opening blocked coronary artery during acute myocardial infarction, in place of thrombolysis is practised in suitable cases. Non-coronary intervention deals with a variety of cardiovascular abnormalities. Subclavian, iliac, femoral and carotid artery balloon angioplasty and stenting are being performed. Vascular dissection and aneurysm repair are also conducted with covered stents. Recently, owing to the advent of several percutaneous interventions, it is being increasingly used to guide these interventions. For example, device closure of ASD, VSD, placement of stent grafts as well as BMV.

Department of Non-Invasive Cardiology

Non-invasive cardiology is relatively new field dealing with cardiac ultrasound as well as various modalities of stress. Coronary Angiography

This is a procedure by which a contrast material is injected directly into the blood vessel (coronary arteries) and is seen by a specialized X-ray machine which takes picture in real time motion. A small tube is inserted through the groin (femoral artery) or through the hand (radial or ulnar artery) which goes up to the origin of coronary arteries and then the contrast material is injected directly into these arteries.

It is a relatively painless procedure which takes 15-20 minutes to complete and the patient can go home in one hour if done through the hand or in 4-5 hours if done through the groin. With the availability of newer generation contrast agents, the chance of developing any serious complication is almost negligible (less than 0.001%). Angiography can tell us how many coronary arteries are blocked and outline the site and degree of block, which helps in planning the further course of treatment like medical management if blockages are less than 50% or angioplasty or bypass surgery if block is more than 50-70%

Beating Heart Surgery

Coronary Bypass Surgeries are performed using contemporary techniques including the off-pump bypass. Over 98% of the surgeries are “off-pump” or “beating heart” surgeries. During off-pump or beating heart surgery, the heart-lung machine is not used. The surgeon uses advanced operating equipment to stabilize (hold) portions of the heart and bypass the blocked artery in a highly controlled operative environment. Meanwhile, the rest of the heart keeps pumping and circulating blood to the body. This offers patients faster post-operative recovery and extracts less of a toll on their systems.

Coronary Interventional Procedures

CABG (Coronary Artery Bypass Graft)

CABG is the shortened more popular name for Coronary Artery Bypass Grafting. Coronary artery bypass grafting is the surgical procedure in which the grafts are connected up to the blocked arteries. This surgery aims to supply the starved heart muscle with more blood by passing the blockages.

The blood vessels from the patient’s own body, which he can spare, are used to perform these bypasses. The internal mammary arteries supply blood to the chest wall and the breast bone. The left internal mammary artery commonly known as the LIMA is used to bypass the LAD, which is the most important artery of the heart. The best results have been observed with use of this artery, which makes this the surgeon’s favourite. The veins from the legs called the saphenous veins and the radial artery from the arm are also used. The surgeon studies the coronary arteries and tailors his plan of revascularization to suit the needs of the individual patient. Once the surgeon joins up or anastomoses the grafts, blood flows through them into the blocked arteries beyond the blocks thus bypassing them. The blocks are not removed but are bypassed, hence the name bypass surgery!
In the Conventional CABG the heart-lung machine is used to perform the operation. The heart-lung machine allows the heart’s beating to be stopped, so the surgeon can operate on a surface, which is blood-free and still. The heart-lung machine maintains life despite the lack of a heartbeat, removing carbon dioxide from the blood and replacing it with oxygen before pumping it around the body.
Beating Heart bypass surgery allows surgeons to perform surgery on the heart while it is still beating. The heart-lung machine is not used.
