Huge Right Renal Tumor With Paraneoplastic Syndrome 

About the Patient

A 58 years old gentleman, presented with Fever, anaemia and sepsis for past 4 months. There are no other comorbities. Patient was symptomatically treated in nearby hospital for past 4 months with no significant Improvement. On examination, patient had fullness in right flank and adviced for imaging..

Case Scenario

On imaging, a solid cystic mass of size 45*30 cm noted in Right kidney extending from Right Diaphragm till Pelvis. Blood parameters were Hb: 6.2, TLC: 32,000 with multiple fever episodes. Patient was explained about the paraneoplastic features of Renal mass and explained regarding the need of surgery after Tumor board discussion. Patient underwent Right Radical Nephrectomy, tumor weighing around 2.05 Kg. Post operatively, Patient WBC counts subsided and anaemia improved and no fever episodes encountered. Post op. period was uneventful and discharged on POD 5. Dr Sathish Kumar Anandan .

Paraneoplastic syndrome incidence in RCC is around 10%, but currently increasing due to increased awareness and overall understanding of the disease. Like our patient, at times it is only presenting feature of RCC.It usually resolves after complete tumor removal.
