Life-Saving ERCP: 80-Year-Old Recovers from CBD Stones & Pus in Bile Duct with high-risk ERCP

Meet Mr. Sanjeev Kumar, grandson of Mrs. Vishnu Bhai, 80 years old, as he shares their experience at Apollo Hospital, Jubilee Hills, Hyderabad. Mrs. Vishnu Bhai was suffering from severe abdominal pain and fever when they came to the emergency department. After evaluation, she was referred to Dr. Ankit Vijay Agarwal, Consultant Gastroenterologist, Hepatologist, and Advanced Therapeutic Endoscopist.

Dr. Ankit Vijay Agarwal explains that after investigations, it was found that Mrs. Vishnu Bhai had Jaundice with elevated bilirubin, liver enzymes, and white blood cells, along with a low platelet count. Initially suspected to have Cholangitis, an ultrasound confirmed she had Acute Calculous Cholecystitis with a dilated Common Bile Duct (CBD) and multiple CBD stones. Dr. Ankit Vijay Agarwal performed an emergency high-risk ERCP (Endoscopic Retrograde Cholangiopancreatography), which showed a grossly dilated CBD. He then conducted a Sphincterotomy and Sphincteroplasty, removing 30 large bile stones and pus from the bile duct.

Mrs. Vishnu Bhai tolerated the procedure well and improved. She later underwent a Cholecystectomy, and during follow-up, her CBD stent was removed. She was discharged in stable condition. This case shows that ERCP can be successfully performed under high-risk conditions with minimal complications.
