    TomoTherapy

TomoTherapy is an advanced procedure that utilises a specialised radiation therapy machine where a slender radiation beam is adjusted as it surrounds the patient, through the device’s tunnel. The term “TomoTherapy” originates from the utilisation of a beam shaped like a strip, ensuring that only a single “slice” of the target receives radiation exposure at any given moment.

The precision, modulation, and full-circle delivery of TomoTherapy make it well-suited for tumours located in challenging areas near vital organs. Additionally, it is effective for treating multiple tumours simultaneously and for retreating areas with minimal side effects.

At Apollo Cancer Centre, Hyderabad, we offer TomoTherapy for cancer patients in need. We are well equipped with recent trends in the medical field, providing us with a niche to provide the best care and excellence to patients.

    Principle Behind Working of TomoTherapy

    • In this treatment method, radiation utilises 3D images acquired before treatment to develop personalised plans.
    • These images help pinpoint tumour size, shape, and location, aiding in dose determination.
    • The TomoTherapy system then calculates precise radiation patterns, positions, and intensities based on prescribed doses.
    • Treatment involves highly targeted radiation beams directed at tumours from multiple angles for optimal efficacy.

    Indications for TomoTherapy

    • Brain cancers and non-cancerous brain tumours
    • Head, neck, and oral cancers
    • Breast cancer, including inflammatory breast disease
    • Lung cancers and tumours in the lungs
    • Skin cancers
    • Prostate cancer, ranging from early-stage to advanced disease
    • Cancers of the oesophagus and upper gastrointestinal tract
    • Cervical, ovarian, uterine, vaginal, and vulvar cancers
    • Colorectal and anal cancers
    • Liver and pancreatic cancers
    • Cancers that have spread to multiple locations
    • Pre-transplant therapy for blood diseases like leukaemia

    How Does TomoTherapy Work?

Treatment Planning: Capturing 3D images of the target area helps clearly outline the tumour contours. It also utilises data from these images; the intensity, position, and shape of radiation beams, which are adjusted accordingly.

CT Imaging and Patient Positioning: Immediately before the session, a CT scan verifies the tumour’s position, and the patient is positioned accordingly for radiation therapy. Occasionally, tumours may alter shape or shift position, warranting a CT scan before each session to ensure precise delivery of radiation to the tumour by specialists.

Radiation Delivery: It helps target the tumour from various angles and operates akin to a CT scanner, revolving around the patient on the treatment couch. This linear accelerator delivers radiation in a spiral pattern, ensuring precise distribution from multiple angles for more accurate tumour treatment.

    Benefits of TomoTherapy

    • Daily imaging allows doctors to monitor tumour shrinkage, enabling precise dose restriction to the tumour while sparing healthy tissue, leading to fewer complications and superior tumour control.
    • Low-dose CT imaging, aided by repetitive imaging, benefits patients with metal implants like dentures or pacemakers, ensuring accurate treatments without artefacts.
    • Achieves high local control of tumours.
    • Minimises radiation dose to healthy tissues and vital organs such as eyes, lungs, heart, kidneys, parotid glands, and others, preserving their important functions.
    • High survival rates and reduced complications or side effects from radiation therapy.
    • Targeted treatments are made feasible by focusing the radiation dose solely on the target or tumour.
    • Provides confidence in managing complicated and paediatric treatments effectively.

    FAQ's

TomoTherapy may lead to short-term side effects such as nausea, fatigue, or skin irritation. Additionally, specific areas treated may experience side effects; for instance, if targeting head cancer, one may encounter headaches, hair loss, or dry eyes. While most side effects typically diminish post-treatment, some may persist long-term or arise months or years later. It is essential to discuss potential side effects and their management with your doctors.

Each TomoTherapy session takes approximately 10-20 minutes. It is non-invasive and as pain-free as a CT scan or X-ray.

TomoTherapy strives to enhance patient comfort throughout the treatment process. Its precise targeting and minimised side effects contribute to a more comfortable treatment experience compared to other radiotherapy methods. Moreover, TomoTherapy sessions are generally more time-efficient than traditional radiotherapy sessions.

Limitations arise with moving targets due to the slice-by-slice dose delivery, which can potentially wash out the dose distribution under certain conditions.
